When our examination of a child’s mouth reveals decayed baby teeth, I’m often asked by the parent, “Why do we need to fix these cavities? These teeth will just fall out in a few years anyway…”

The primary reason for restoring damaged baby teeth is that they are important in the development of your child’s mouth and to the healthy and correct transition to the adult dentition that will follow at a later date. Baby teeth are present in the child’s mouth for reasons beyond their usefulness to eating. They need to stay in place until the permanent teeth that will replace them are in position to erupt into the mouth.

The presence of both baby and permanent teeth has a supporting effect on the bone of the jaws. Early loss of baby teeth can lead to loss or weakening of the surrounding supporting bone and can compromise the eruption of the permanent successors. The child’s first permanent molars are called six-year molars because six years old is about the time these early erupting permanent teeth appear. Baby teeth serve to guide these permanent teeth into their correct positions. If the child loses baby teeth, these early-erupting permanent teeth will erupt into a forward position, drifting and tilting into the spaces where the baby teeth were lost. This will create crowding conditions that can only be corrected by later orthodontic therapy.

Baby teeth begin to erupt in infancy at around 6 months and all twenty baby teeth should be erupted around age two years. It’s a good idea to have your toddler regularly examined by a dentist or hygienist to check for cavities or developmental abnormalities. The dentist can show you how to clean your child’s mouth properly and how to prevent “bottle baby syndrome”, a condition of aggressive decay of the child’s upper front teeth. He can advise you on the role of supplemental fluoride and why its use in early years can be so beneficial in developing strong permanent teeth.

Here are some tips from the American Dental Association:
Preventing Baby Bottle Tooth Decay
• After each feeding, wipe your child’s gums with a clean, damp gauze pad or washcloth.
• When your child’s teeth come in, brush them gently with a child-size toothbrush and a small smear of fluoride toothpaste.
• Parents should be in charge of ensuring plaque removal with the toothbrush until the child achieves the manual dexterity to properly brush his teeth.
• Parents should continue to supervise brushing until the child can be counted on to spit and not swallow toothpaste—usually not before he or she is 6 or 7.
• Place only formula, milk or breast milk in bottles. Avoid filling the bottle with liquids such as sugar water, juice or soft drinks.
• Don’t put your baby to bed with the bottle. Infants should finish their bedtime and naptime bottles before going to bed.
• If your child uses a pacifier, provide one that is clean—don’t dip it in sugar or honey.
• Encourage your child to drink from a cup by his/her first birthday.
• Encourage healthy eating habits.

When your child’s baby teeth begin to erupt, talk to your dentist about when to schedule a well-baby dental checkup. Early establishment of good habits is key to a lifetime of good dental health for your child.
